DIAGNOSTIC TEST SCHEDULE

Thank you for considering MICA in the education of your child. 

Please read the instructions before filling out the form:

1. Student Applicant for Levels 1 to 10 is required to take the Diagnostic Test.

   1. submit the following:  

      1. photocopy of child’s birth certificate,

      2. photocopy of the recent Report Card,

      3. Certificate of Good Moral Character;

   2. pay for the Diagnostic Test fee, then know your child’s schedule for the said test,

   3. know when will be the Interview for both the student and parents,

   4. after the Interview, know the Enrollment Procedure. 

2. A student coming from schools using the A.C.E. curriculum or School of Tomorrow system

   1. submit the following:  

      1. photocopy of child’s birth certificate (PSA),

      2. photocopy of the recent Report Card,

      3. photocopy of Good Moral Certificate,

   2. know when will be the Interview for both student & parents,

   3. after the Interview, know the Enrollment Procedure.

3. Requirements for Kinder 1, Kinder 2 and Grade 1

   1. Kindergarten I

      1. must have turned 4 years old on or before October 30,

      2. submit a photocopy of the child’s birth certificate (PSA),

      3. know when will be the Interview for both student & parents,

      4. after the Interview, know the Enrollment Procedure.

   2. Kindergarten 2

1. must have turned 5 years old on or before October 30

2. same requirements listed above for  Levels 1 – 10 (Item I)

1. Grade 1

   1. must have turned 5 years old within the recent SY 2019-2020 when he/she completed the Kindergarten 2
